Reporting requirements, including due dates, are clearly articulated in each grant agreement. All multi-year grants are contingent upon performance criteria being met. A final report is required for each grant. Reporting Guidelines are included with each grant agreement.
Grantees wishing to amend a project’s objectives, activities, budget and/or timeline should contact their program officer or the Grants Manager to discuss the desired changes then
- Submit requested revisions to objectives, activities and/or timeline in writing via e-mail.
- Submit requested budget modifications using the Budget Modification Form
For multi-year grant Final Reports, provide final year and cumulative program charts and financial reports. If a balance is reported and an extension is required, please submit the request in writing along with a proposed budget for the use of unexpended funds using the Proposed Budget for Unexpended Funds Form.
