UniHealth Foundation recognizes that our grantees may wish to amend a project’s objectives, activities, budget, key personnel, timeline, or any combination of these in order to better achieve a program’s goals. If this is the case, please contact your Program Officer (whose contact information is included in your award letter) to discuss your intended post-award changes.

Please expect to:

• Submit your requested grant modifications in writing via e-mail.
• Submit your requested budget modifications using our Budget Modification Form.
