How to Submit a Report

General Reporting Guidelines

Your UniHealth Foundation grant agreement includes designated reporting requirements for your project, including all due dates. If your grant is a multi-year grant, the subsequent payments of the grant are contingent on the submission of the report, the program staff review of the report, and the approval of the release of the payment. Submission of a report is not a guarantee of immediate release of payment.

Final Reports

A final report is required for each grant. If you are providing a multi-year grant final report, please include final year and cumulative program charts, as well as financial reports.

If you are reporting a balance and require an extension to your grant period, please submit the request in writing along with a proposed budget for the use of unexpended funds.
